This feature is no doubt what PCGS is referring to in its "Repaired" designation - however the fields were smoothed at time of issue to remove imperfections and make the medals more appealing to the recipients and elite purchasers of these pieces, and can hardly be regarded as repairs or damage. Many of these medals were made with a suspension loop at the top, and it is possible this example at one time was looped - but that cannot be ascertained because the edge cannot be seen in the holder.
